Kingsway Indoor Stadium Membership Suspension Form

If you wish to suspend your membership at Kingsway Indoor Stadium, please read the terms and conditions below and then proceed to completing the online form.

1. The membership suspension form must be completed at least 48 hours before the suspension begins.
2. Membership suspension is not available for the 30 days for 30 dollars ($30) membership.
3. The following minimum and maximum membership suspension periods apply:
   • 3 month membership: Minimum one week, maximum four weeks
   • 12 month memberships Minimum one week, maximum eight weeks.
4. Exceptions to point 1 and 2 above applies if you have a valid medical certificate.
5. A membership may have a backdated suspension period added if the member is able to produce a medical certificate for that period.
6. To reduce or extend a suspension period, Kingsway Indoor Stadium must be informed via email prior to the suspension period’s completion date – kingswayindoorstadium@wanneroo.wa.gov.au.
7. The Health and Fitness Team Leader retains absolute discretion in the granting of membership suspension periods, included backdated suspensions.
8. If the suspension is approved:
   • Direct Debit payments will be paused or amended according to the suspension dates. Payments may be amended again if any subsequent changes are made to the dates.
   • If you paid your membership upfront, the membership will be extended in accordance with the maximum periods permitted.
