Kingsway Membership Cancellation Form

If you wish to cancel your membership at Kingsway Indoor Stadium, please read the important information below and then proceed to completing the online form.

Important information

Direct Debit Memberships

• If your membership cancellation request is approved your membership and all direct debit payments will cease with one calendar months’ notice. A final payment may be debited as a result.
• Termination of membership will be completed once the member has met all minimal fee obligations and paid all outstanding arrears.

Paid Upfront Memberships

• If an upfront membership cancellation is granted and a refund is approved, the member will be refunded a pro-rata amount from the date of the application request. (Date that this form is submitted).
• The Health and Fitness Team Leader retains the absolute authority to grant or reject any refund or cancellation application.
